Care Management

 

ADIA provides all clients with a dedicated Care Management team to provide the maximum guidance resources for families and their loved ones.

ADIA’s Friendship Care Coordinators are trained to help older adults and/or their families care for older relatives, while encouraging as much independence as possible.

Our Care Management Services are based on providing the maximum guidance and resources for families of older adults and others with chronic needs, such as Alzheimer’s disease, Parkinson’s, cancer and stroke.

We will assign the client and their family with an experienced Friendship Care Coordinator who will be your primary contact. Whenever you need help or have questions, you will have a direct line to contact us 24 hours a day, 7 days a week.
